Teleporting ships

Anyone else see ships occasionally teleport to random, distant parts of the sector?  My survey ship suddenly teleported to outside its travel range, but it still says it's surveying . . . huh?  This also happens to regular ships sometimes.  It's a rare bug but I've see it happen a few times.

To be clear, my survey ship finished with an anomaly and I moved it onto the next one.  Then I clicked the ! and made my anomaly choice.  Then my ship was suddenly far from where it had been previously.
